On this page you will find three ways to initiate the Application Process – online, by postal mail/fax and a hybrid of these two. Please read each option carefully before deciding which method works best for you and beginning the application. Once you have chosen a method, click on the link to begin the Application Process.
With all three methods, you will need to complete all forms, as well as provide 2 recent photos, copies of transcripts and ordination certificate (if applicable), and either a $100 application/background investigation fee or proof of a current criminal background check.
PLEASE NOTE: If you need a copy of your DD-214, and do not have it readily available, the National Personnel Records Center (NPRC) has provided the following website for Veterans to gain access to their DD-214 online: Request Military Service Records
We require copies of DD-214s of all veterans as part of the application process.
Application Method I – Online
To initiate the Application Process online, please follow the link below.
In order to complete this process, you will need to have digital copies of 2 recent photos, transcripts, ordination certificate (if applicable), DD214 (for prior service-members) and proof of a current criminal background check prepared and ready to upload. If you do not have proof of a current criminal background check, you will be prompted to pay your application/background investigation fee of $100 via PayPal. All documents for upload should be in either PDF or JPG format.

Application Method II – Hardcopy by Postal Mail/Fax

All the documents need to be completed and sent with 2 recent photos, transcripts, ordination certificate (if applicable), DD214 (for prior service-members) and either a $100 application/background investigation fee or proof of a current criminal background check to:

Application Method III – Partial Electronic — Partial Postal Mail/Fax
To initiate the Application Process by both postal mail/fax and electronic application, please follow the link below.
In order to complete this process, you will need to complete the electronic application and download a 3-page PDF document that requires your signature. Your signed forms, along with 2 recent photos, transcripts, ordination certificate (if applicable), DD214 (for prior service-members), and either a $100 application/background investigation fee or proof of a current criminal background check should be mailed to:
